I understand fitness and nutrition programs are developed individually to reflect my specific goals and copying and/or redistributing said programs or any content Complete Physique® sends directly to me or found on their website for use other than my own is prohibited and unlawful.
I understand by becoming a member of Complete Physique® I am agreeing to scheduled monthly payments of $49.99 each month (Basic Member) or $79.99 each month (Premium Member). I understand these payments will be automatically deducted from my account on the same day every month.
I acknowledge that all fitness and nutrition programs and activities carry with them the potential for personal property damage, personal injury, and/or death.
I assume all risks of injury arising out of my participation in the exercise activities with Complete Physique® LLC and Nancy Bond.
I certify that I am physically fit and of good health to be able to participate in fitness and nutrition programs and activities and have not been advised otherwise by a qualified medical professional.
I release and agree to Complete Physique® LLC and Nancy Bond from any and all liability, loss, damage, expense, or cost of any nature whatsoever for any and all claims that are known or unknown, foreseen or unforeseen, future or contingent, for personal injury or property damage arising out of exercise and nutrition activities.
I agree that I shall not now or at any time in the future, directly or indirectly, commence or prosecute any action, suit or other proceedings against Complete Physique® LLC and Nancy Bond arising out of, relating to, or in connection with my participation in exercise and nutrition activity.
I consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during my participation in exercise and nutrition programs and activities with Complete Physique® LLC and/or Nancy Bond.
I attest that I am of legal age to sign this form as a binding legal document in accordance with its intention. I have carefully read this release of liability and fully understand its contents. I agree to assume all risks of injury associated with all activities with Complete Physique® LLC and Nancy Bond, and I agree not to make a claim against Complete Physique® LLC and Nancy Bond if I am injured while engaging in exercise or nutrition activities. I sign this agreement of my own free will.
